With primary care under pressure to meet rising patient demand and frequently deliver more with less, Livi’s new care provision model offers an all-in-one clinical and tech solution to enable practices to operate more efficiently.
The new service from the UK’s leading digital-first healthcare provider, Livi, will serve as a ‘Virtual Practice Partner’ to Integrated Care Systems (ICS), Federations, Primary Care Networks (PCN) and GP practices across England. It combines clinical resources and appointment support with digital tools including patient app, remote monitoring, smart messaging and websites.
Digital access routes and technology solutions form an essential part of the UK Government’s long term plan. It aims to ensure that digital transformation plans enhance primary care delivery by improving access to vital appointments and joining-up care services to better plan for future patient needs.
By bringing together digital and in-person healthcare services across primary and secondary care, Livi delivers an efficient and personalised patient experience. The new model will continue to support QOF (Quality and Outcomes Framework) attainment and reduce physical and digital DNAs (Did Not Attend).

Digital consultation tool: enables practice GPs to seamlessly work in a physical and digital care environment
Digital capacity on demand: access extra clinical capacity via Livi’s 450+ GMC-registered NHS GPs delivering consultations, easily managed via Livi’s user-friendly booking tool
Tailored patient app for NHS partners: enabling patients to easily book and manage appointments with the capability to message a Livi GP
Consultation reminders: practices can send automatic reminders directly via text message and NHS app for physical or digital meetings, minimising no shows
Remote patient monitoring and patient questionnaires help monitor and capture crucial data on the health of their patients, and support QOF points attainment, thereby reducing unnecessary healthcare interactions
Secure messaging: practices will be able to send bulk messages to specific cohorts of patients, allowing patient responses to be coded into your clinical system and support practices to reach underserved populations or those living with chronic disease
